General Conditions
University travel policy is located at http://www.unimelb.edu.au/ppp/docs/14.html#14.1.
Staff and students who travel on official business are responsible for complying with this policy.
Staff and students travelling overseas should consult the Considerations for Staff Travelling overseas
All travel must be approved by the Head of Department PRIOR to departure. Please submit the appropriate forms as given in Table 1 to:
Staff - to HOD via Fanoula
Post-graduate students –to Postgraduate Co-ordinator via Fanoula.
Honory Staff - to HOD via Fanoula

*1. HR18 Forms
Completing an HR18 form ensures the Staff member is covered by University travel insurance (details of the policy can be found on the Risk Management Office website). Every trip must have a completed HR18 form authorised BEFORE travel is taken. HR 18 forms can be downloaded from:

*2 Guideline for Off Campus Travel and Work
All staff should consult the Environment Health and Safety Manual Off Campus Work and Travel Policy to ensure work and travel by University staff and students at off campus locations is undertaken in a safe manner.

Any assistance for Risk assessments can be obtained form Trudi Harris. Generic risk assessments for travel by car or plane are available and do not need to be done.
*3 Academic Checklist
All academic staff who have teaching and supervisory responsibilities are required to complete the Academic Responsibilities Checklist Form and submit it with their leave forms.
*4 Travel Diaries
Completion of a travel diary is compulsory for all overseas travel and travel within Australia for 5 nights or more (see Table 1.). For students who have been employed casually by the University in any capacity must complete a travel diary as required in table 1.
Your travel diary must contain the following information in respect of each business activity:
● The date and approximate time when the activity commenced
● The place where the activity was undertaken
● The duration of the activity; nature of activity
● Costs associated with the activity
● Your Name, Signature and declaration “ l declare this record correct”
A diary may be hand-written or typed. A typed diary must be printed and authorized by the employee. Other documents such as a conference schedule may be transformed into travel diaries.

*You can delete the entries in this example and use this as a pro-forma for your travel diary
Travel diaries must be submitted within 14days of completion of travel.
*5 StudentTravel Forms
Students travelling on University business need to complete the Student Travel Insurance-Registration Form.
This will provide free insurance cover for them while travelling.
This form needs to be submitted to the Insurance office, 1 week prior to travel, to ensure you are covered.
Students may apply to the Department for travel support when presenting at a conference.
● Honour Students may apply for ASCEPT registration and conference dinner costs (dependent on being ASCEPT members)
● PhD students are able to apply for $500 in year 1 of canditure for conference support.
$2500 in year 2 for overseas conference and lab visit support.
$500 in year 3 of canditure for conference support.

Safety Induction for Temporary Visitors
- Provide temporary Visitor with a “Visitors Emergency Guide”, available from Reception and from Property and Campus services: http://www.pb.unimelb.edu.au/emergency/publications/visitors/visitors.html
- If visit duration greater than one day, then treat like staff/student member of Department:
- Show visitor where to obtain safety information:
- Noticeboards, evacuation and first aid signage, department website.
- Local lab group information, such as MSDS, risk assessments if completing laboratory tasks.
- Check if requirements are needed for other monitoring for specific hazards such as radiation monitoring or other personal monitoring.
- Provide appropriate PPE where needed.
Accompanied at all times? Yes, then Induction complete.
IF NOT :
- Complete risk assessment with visitor
- Ensure adequate training (protocol instructions, risk assessments) , supervision and documentation of training/competency is completed.
